HMO Plans: HMO plans generally require insurance authorization. You will be billed at the self-pay rate if you are seen without a valid referral on file. Please confirm your authorization before your visit.
Understanding Your Patient Responsibility
Your financial responsibility is determined by your insurance plan. We follow your insurance plan's decisions regarding deductibles, copays, and coinsurance. We do our best to estimate costs upfront, but the final amount is confirmed after your insurance processes the claim.
We will send you an email invoice after insurance claim processing — please pay on time. We provide services in good faith that balances will be paid. If you overpay, we keep the amount as a credit for future visits. Contact us if you prefer a refund.
Medicare vs. Medicare Advantage
If you have enrolled in a Medicare Advantage plan, it is extremely important that you provide us with that information at your visit. While you will always retain your traditional Medicare number, you must use your Medicare Advantage plan information when visiting our clinic.

We offer competitive self-pay rates for patients without insurance or those seeking straightforward care. Fees are based on complexity of the visit.
